Work Permit Information

 WORK PERMIT INFORMATION

Students must have a job in order to get a work permit.

Should say- Work Permits will be issued through the Amador Valley High Career Center Monday - Friday 7:30am-4:00pm.

 

Items needed for regular work permit applications include proof of age (picture ID or birth certificate) and proof of school enrollment.

 

You must pick up the form –STATEMENT OF INTENT TO EMPLOY MINOR AND REQUEST FOR WORK PERMIT available in AVHS Career Center or in the main office or at the Adult Education Office.  This form needs to be filled out by your employer and signed by your parent.

 

Do not forget your social security number.  Also be sure your employer prints their name and gives the business phone number.

Be sure you allow enough time for the process to be completed!
