We welcome you to Amador Valley High School's On-Line Walk-Through Registration Packet. Please read the instructions below carefully. There are several links to information that students and parents must read, and forms that must be filled out and signed BEFORE arriving at the school for the physical walk-through registration. Current Tdap vaccine required for the 2011-2012 school year, given on or after 7th birthday, if you have not yet provided proof, it is required to register. All forms on this web site are downloadable (Microsoft Word Format), and available for completion on the computer. Items for walk-through registration can be purchased ahead of time online through Amador's On-line Web store and breeze through the payment line at the walk-through registration. Please follow these instructions to complete your walk-through packet.

Packet Procedures: There are three (3) organizations that are distributing and collecting information during the registration process:

  1. Amador Valley High School
  2. AVHS Parent-Teacher-Student Association (PTSA)
  3. AVHS Athletic Boosters Organization.

AMADOR VALLEY HIGH SCHOOL FORMS AND INFORMATION

READ the following items in order before completing necessary walk-through forms:

  1. Pleasanton Unified School District (PUSD): Annual Notice to Parents Regarding Federal Laws and Education Code Relating to Rights of Parents or Guardians of Minor Pupils. (English) (Spanish)
  2. Amador Valley High School (AVHS): Discipline Plan.
  3. No Child Left Behind Act: Directory Information Notification.
  4. AVHS: Notice to All Transfer Students. (Transfer Students Only)
  5. Student Medical Insurance Coverage Registration. (English) (Spanish) (Flyer)
  6. AVHS: Health Office Procedures information.
  7. PUSD: Senior Ad Purchase Information (Seniors Only Please!).

The following forms and information are REQUIRED to complete the Walk-Through Registration Process.

You may use your keyboard/computer to fill in and PRINT each of the following forms.

These forms will be collected at the beginning of registration starting in the AVHS Multi-Purpose building

  1. Show two (2) Proofs of Residency. PUSD currently acknowledges the following as acceptable proof for residency (California Drivers License IS NOT an acceptable proof of residency):
    * Current utility bill (complete bill) with parent and/or legal guardian's printed name and address (includes PG&E, water, garbage, or cable TV with service address listed).
    * Land line telephone bill (no cell phone bills please).
    * Settlement statement or rental/lease agreement:
    YOU MUST HAVE THESE TWO (2) PROOFS OF RESIDENCY WITH YOU OR YOU WILL NOT BE ABLE TO CONTINUE THE WALK-THROUGH PROCESS
  2. Current Tdap vaccine required for the 2011-2012 school year, given on or after 7th birthday, if you have not yet provided proof, it is required to register.
  3. TURN IN the Emergency Card you have picked up from the school having made any changes or additions to the personal information needed on the form. (Mandatory Emergency Card Pick Up in AVHS Main Office August 10-12 8:00am-3:00pm)
  4. Turn in a completed and signed Walk-Through Registration Signature Page.
  5. Turn In Healthy Kid Survey Consent Form (FOR 9th & 11th GRADE ONLY).
  6. PRINT the Picture Order Form if you would like to purchase your child's school photograph. (OPTIONAL)

THE FOLLOWING STEP WILL TAKE PLACE AT THE STUDENT ACTIVITIES OFFICE Or ON-LINE

PAY: This is your opportunity to buy an ASB sticker (gets student into sporting events free, and discounts on tickets and school items like yearbooks), Yearbooks, parking passes (Juniors and Seniors Only during the Fall semester), and Class T-Shirts. There are two payment options this year.

  • Pre-Pay On-Line - No Convenience Fees!

    PRINT YOUR RECEIPT AND BRING IT WITH YOU TO REGISTRATION. Doing so will allow you to SKIP the payment line during the walk-through registration process.

  • If you prefer to pay in person please print out this payment form: AVHS Walk-Through Payment Form and visit the activities office to make your payments.
